Deanna: Hi, my name is Deanna Clark-Esposito and I'm with the Clark-Esposito Law Firm, P.C. I'm here with Jacob Lundy and we're here to talk about what you can expect to find in our questionnaire. Jacob, can you explain to everybody what's involved with our questionnaire? 

Jacob: Oh, sure. I'm Jacob Lundy with the Clark-Esposito Law Firm, P.C. in New York. And I'm sure we've spoken by phone and email a couple of times before. So we have attached to this email our internal questionnaire and we send it out to prospective new clients and it's a little bit of record keeping and some of the information is actually required for us to obtain before consultations by New York State law. It gives the Managing Attorney a chance to look over some of the details of your company, your questions, concerns, and goals. And it of course has important terms of service for the consultation. Most of the boxes are compulsory. So it'll prompt you to complete all of the boxes beforehand and you just click submit and it comes back to me and I'll usually follow up with confirmation that we've received it. 


Deanna: Okay, terrific. Now this comes electronically. Is there a paper version? Tell us about the format. 


Jacob: Sure. So it is an electronic document. You'll receive it from the firm's email address, which we've mentioned in some earlier communications, I'm sure to add you to our contacts list just so we can ensure that you received things. You can always reach out if you think you've missed the questionnaire or have any problems with the document. I can actually help filling out the questionnaire with prospective new clients over the phone. So if you have any connectivity issues or problems with the document, I would be happy to help fill the questionnaire out with anyone.
